At Tom of P-Town Health, we talk about everything—because your health, your body, and your sex life deserve honesty and science. Today, we're diving into Trimix and Bimix, injectable medications used to treat erectile dysfunction (ED), especially in men who don’t respond well to pills like Viagra or Cialis.
These meds are powerful—sometimes shockingly so—and while they can be incredibly effective, they come with risks that you should know about before using them.
π What Are Trimix and Bimix?
Trimix is a compounded injectable medication that includes:
Alprostadil – a prostaglandin that increases blood flow
Papaverine – a smooth muscle relaxant
Phentolamine – an alpha-blocker that dilates blood vessels
Bimix contains just papaverine and phentolamine, and skips the alprostadil. This makes it slightly less potent and sometimes better tolerated for beginners or those prone to sensitivity.
These medications are injected directly into the erectile tissue of the penis (the corpus cavernosum) using a small, insulin-sized needle. Erections usually occur within 5–15 minutes and can last 30–90 minutes.
π§ Why Use Injections Instead of Pills?
Injectable ED meds bypass the need for arousal, stimulation, or even a psychological connection. You can go from zero to sixty regardless of mood, emotional state, or chemistry. For this reason, they are often used by:
Sex workers
Porn actors
OnlyFans and content creators
Men with performance anxiety or severe ED
This “on-demand” capability can be a game-changer for people who need a reliable erection, regardless of circumstance.
⚠️ The Risks: What Can Go Wrong
π¨ 1. Priapism (An Erection That Won’t Go Away)
If the dose is too high or your body overreacts, you can end up with a painful erection that lasts longer than 4 hours. This is a medical emergency. Left untreated, it can lead to permanent tissue damage and even erectile dysfunction.
If you're worried about priapism, try this first:
Do 20–30 minutes of vigorous exercise like running or squats.
Apply an ice pack to the inner thighs (not directly to the penis) to reduce blood flow.
If it's been more than 4 hours, or pain is increasing, go to the ER immediately and tell them you used a vasoactive injection for ED.
𧫠2. Contaminated Injections
Trimix and Bimix are compounded medications made by specialty pharmacies. That means they aren’t regulated the same way as mass-manufactured drugs. If a pharmacy cuts corners, your medication could be contaminated, leading to infections, abscesses, or even sepsis.
π Has Your Compounding Pharmacy Been Written Up by the FDA?
To find out if a compounding pharmacy has had issues with sterility, contamination, or regulatory violations:
Go to the FDA’s Inspection Database:
π https://www.fda.gov/inspections-compliance-enforcement-and-criminal-investigations/inspection-references/inspection-classification-databaseSearch by pharmacy name or location.
If they’ve been issued a Form 483, a Warning Letter, or have had their outsourcing facility license suspended, it will show up here. You can also check the FDA’s Compounding Pharmacy Warning Letters page:
π https://www.fda.gov/drugs/human-compounding/compounding-pharmacy-warning-letters
Avoid any pharmacy with unresolved sterility concerns or recent citations. If in doubt, ask your provider to help vet the pharmacy.
π Alternatives to Trimix and Bimix: FDA-Approved Options
If you're not comfortable with compounded meds—or want a standardized, pre-filled option—consider these:
Edex
Pre-filled syringe of alprostadil, made under FDA-approved sterile conditions.
Less risk of contamination or variability in dosing.
Slightly more expensive, but safer for those wary of compounded meds.
Available in 10mcg, 20mcg, or 40mcg doses.
Caverject
Also contains alprostadil in a pre-measured, injectable system.
Comes in both syringe and dual-chamber pen systems.
More convenient and consistent than compounded alternatives.
Muse
Urethral suppository form of alprostadil.
Not as potent, but no needles involved.
Good for men with mild-to-moderate ED or needle aversion.
Oral ED Meds
Still a good starting point or adjunct:
Sildenafil (Viagra) – best taken on an empty stomach
Tadalafil (Cialis) – longer-acting, can be taken daily
Vardenafil (Levitra, Staxyn) and Avanafil (Stendra) – newer options with varying profiles
You can even combine oral meds with injectables (under supervision) to use lower doses of each.
π‘️ Tips for Safe Injection
Use a 31G insulin syringe, ½ inch length.
Clean the area well with alcohol prep beforehand.
Inject at 10 or 2 o'clock position on the shaft—never on the top, underside, or head.
Alternate sides with each use to avoid scar tissue.
Max usage: 3 times per week, 1 injection every 24 hours.
Don’t reuse needles. Ever.
We’ll happily walk you through the technique in the clinic. You won’t be the first—or the last—guy to ask.
π³️π Bottom Line
Trimix and Bimix can restore sexual confidence, autonomy, and spontaneity—especially for those who haven’t had success with pills. But they’re not toys. Injectables come with serious risks if misused or sourced from shady compounding pharmacies.
Want help choosing the right ED treatment or finding a reliable pharmacy? That’s what we’re here for.

You deserve great sex and great health.
